The Website Service Desk (Field) module, developed by the Center of Research & Development, enables integration between the Website Service Desk and Generic Request (Fields) modules, allowing the use of additional fields when submitting requests via the website interface.

Additional fields are pre-configured by the administrator. More information on the fields setting up you can read in the instruction to the Generic Request (Fields) module.
